PUPPY HEALTH GUARANTEE: 

As we cannot cover every possible health issue on this form, we do encourage you to reach out to us if you have any health concerns or issues and we will assist you in any way that we can.  We pride ourselves on having a strong, healthy breeding program and like to know about any issues so we can adjust our breeding program as needed.   

NON-BREEDING AGREEMENT: 

This will need to be filled out to send in with the CKC registration for puppies purchased as PETS.  If you are purchasing breeding rights this does not apply.
